Finster Commissioner, Precinct 4 Wendy Marvin Deputy County Clerk Thereupon the following proceedings were had: Commissioner Galvan gave the Invocation and Commissioner Finster led the Pledge Allegiance. GENERAL DISCUSSION: Mark Crippen gave an update on the New Jail Construction and as of now the jail schedule is proceeding on schedule. There were no other weather related delays in the month of July and we are almost through the month of August and it looks like there will be no other delays during August. The completion date is set for August 3, 2005. The financial side of the construction is • still looking good. There are six changes within the budget that were incorporated into the estimate, and two of the changes are City related. Commissioner Balajka and Mark Crippen will visit with the City about these issues when the changes are fully defined and the prices are completed. Courthouse Renovation Update; within the next two weeks the Department Heads will be talked with regarding the relocation area at Memorial Medical Center, April 2005 is currently the date they are scheduling for the move. The duration of the stay at Memorial Medical Center is 12 to 14 months. Mark hopes to have on the Agenda for next Commissioners' Court Meeting the approval of Bailey Architects as the architect engineering team for the Courthouse Renovation/Restitution Project, APPROVE THE INDEPENDENT AUDIT REPORT FOR THE YEAR ENDED DECEMBER 31 2003 FROM RUTLEDGE CRAIN & COMPANY PC CERTIFIED PUBLIC ACCOUNTANTS' Louis Crain with Rutledge, Crain & Company, PC presented the audit report for the year ending December 31, 2003. A Motion was made by Commissioner Finster and seconded by Commissioner Floyd to approve the independent audit report for year ending December 31, 2003 from Rutledge Crain & Company, PC, Certified Public Accountants. Commissioners' Galvan, Balajka, Floyd, Finster and Judge Pfeifer all voted in favor. • A Motion was made by Commissioner Floyd and seconded by Commissioner Balajka to approve revisions to the plat of Sunilandings Phase I & Enchanted Harbor #1 as indicated on G & W Engineers Drawing; File #3567-001 Sheet 1 of 1 and authorize Judge Pfeifer and Commissioner Floyd to sign drawing. Commissioners' Galvan, Balajka, Floyd, Finster and Judge Pfeifer all voted in favor. SEE ATTACHED DRAWING 45 L MEDICAL CENTER— MONTHLY IFxNANCIAL The Memorial Medical Center monthly financial report was presented by Saad Makhail. Buzz Currier, Hospital Administrator stated that the hospital became a critical excess hospital effective August 1, 2004. The hospital is continuing recruiting for the Port Lavaca Clinic and for Dr. McFarland. Mr. Currier stated they have received a grant from ORCA for $45,000 with a 10% matching obligation that the hospital will put in. The grant is to purchase equipment to update ICU, Med-Sung and the Emergency Room. He also stated that they are please to announce they have found a replacement for Saad who Is leaving at the end of September his name is Jamie • Jacobe a Certified. Public Accountant with about 15 years experience in health care. A Motion was made by Commissioner Floyd and seconded by Judge Pfeifer to renew for year 2005 the contract with Golden Crescent Communications for maintenance of Calhoun County's Emergency Communication Equipment and Systems. RENEWAL RATES FOR THE COUNTY' INSURANCE WITH BLUE CROSS/BLUE SHIELD: John Debler with GSM Insurors gave a report on the comparison of the 2 tier and 5 tier rates. If the County was to keep the 2 tier rate structure the monthly premium would-be $126,473 and if the County was to go with the 5 tier rate structure the monthly premium it would be $123,623 with a monthly savings of $2,850 and an annual savings of $34,200 this is if the current employee contribution levels are maintained. Even if the 5 tier rate structure is adopted, you can still have each employee with dependents pay the same as they are paying now. The contributions do not have to change to reflect the 5 tier structure, but they can if the County should choose to. For new employees who have to pay the entire cost of the dependent coverage, the 5 tier structure may make It possible for some to afford the coverage, especially if they have one child or a spouse only. John stated that he can see no negatives with changing the rate structure from 2 tier to 5 tier and this would be his recommendation. A Motion was made by Commissioner Galvan and seconded by Commissioner Balajka to go with the 5 tier structure plan with no change leaving everything the same, with the effective date as October 1, 2004. Port Lavaca, Texas, hereinafter referred to as "affiliate" and The Victoria College Victoria, Texas, hereinafter referred to as "college", that the field internship experience for students enrolled in the Emergency Medical Service Professions Program at The Victoria College will be provided at Calhoun County E.M.S.. It is agreed that the college and the affiliate will jointly select and assign learning experiences. The college will provide the service with a list of learning objectives and schedule of student assignments prior to the beginning of each experience. The Victoria College and Calhoun County E.M.S. are affirmative action/equal opportunity institutions in regards to all programs and activities. No discrimination shall be made between individuals with regard to race, color, religion, national origin, sex, age, or handicap. The field internship is a period of supervised experience on an EMS vehicle which provides the student a progression of increasing patient care responsibilities proceeding from observation, to supportive roles, and finally acting as a team member. The affiliate must be licensed by the Texas Department of Health as an EMS Provider and the vehicle must be staffed, equipped, and operate at least at the level the student is in training; i.e.. BLS vehicle for EMT -Basic students, ALS for EMT -Intermediate students, and MICU for Paramedic students. The student must at all times be under the direction and supervision of an assigned preceptor. The preceptor must be currently certified to at least the level of which the student is in training. The initial student role shall be that of observer. After participating in actual patient care, the student may finally function as a patient rare leader. However, the student shall not be placed in a position of being a necessary part of the regular EMS team. The team shall function without any necessary use of the student. Field Internship Affiliation Agreement THE VICTORIA O .F REspoW IBI ITI The Victoria College EMSP Program agrees to: a. Place only those students who are registered and academically qualified for internship experience for the purpose of learning in the field setting. • b. Confer with the affiliate's administration, prior to registration for each semester, concerning the assignment of students to the service. C. Provide the affiliate with a detailed schedule including student name, date & times of assignment, and level of student training. d. Conduct administrative and educational activities of the EMSP Program according to the established policies of the college. These include provisions for students health; keeping records of students' experiences, such as records of rotations, attendance, and proficiency; and evaluation and counseling of students with regard to performance. e. Guarantee that each student has a report of current immunizations and TB test results. The report shall include immunization history for diphtheria/tetanus, measles/mumps/rubella, and if applicable, HBV. TB tests results must be within one year of program entry and must be repeated annually throughout the program. f. Guarantee that each student and faculty member has professional liability insurance coverage. Terms of coverage or copies of the policy are available at the Allied Health Division office at the college. g. Guarantee that each student and faculty member agrees to abide by the policies • and procedures as set forth by the affiliate and the college. h. Guarantee that each student and faculty member has successfully passed training on the Health Information Portability & Accountablilty Act. i. Allow use of teaching equipment owned by the college to be used in the affiliate's in-service training program, so long as the equipment is not needed to fulfill the college's training obligations. The affiliate will be responsible for damages as a result of negligence. Program faculty may assist in in-service education whenever mutually agreed. j. If a student sustains injury, sudden illness, or exposure to communicable disease or bodily fluids occurs during performance of duties; said injury, illness, or exposure will be handled according to college and affiliate policy. See the college's Student Handbook and Allied Health Infection Control Policy. The student is responsible for any cost of medical treatment. 1W Field Internship Affiliation Agreement AFFILIATE RESPONSIBILITIES Calhoun County E.M.S. agrees to: a. Provide for EMSP Program students, without compensation to the student, assignments to the affiliate for purpose of field internship experience. Provide and assign preceptors for each student experience. b. Provide overall control and supervision of patient care. C. Provide orientation of assigned EMT Program students. d. Maintain a qualified staff, vehicles, and equipment as required by the Texas Department of Health. e. Evaluate student field performance and communicate the results to the EMSP Program Clinical Coordinator. The field preceptor is the person responsible for overseeing the student's activities during an ambulance rotation. Preceptors must be certified to at least the level of certification the student is currently seeking. Preceptors will be jointly approved and oriented by the college and the affiliate. The preceptor does not have to be present during basic, non-invasive care, but must direct and witness any invasive procedure. Preceptors are also responsible for evaluating student clinical performance and reporting to the college's EMSP Program Clinical Coordintor. A list of approved preceptors will be maintained by the affiliate and the college. SCHEDULING Date and time perimeters will be finalized prior to scheduling any rotations. If the affiliate has or establishes any other affiliations, assignments will be coordinated to avoid duplication of schedules. Students will schedule rotations through the college's EMSP Program Clinical Coordinator according to date/time perimeters set by the affiliate, preceptor availability, and their personal schedule. The college will submit to the affiliate, a schedule indicating the date, times, student name, and training level. IERMS OF AGREEMENT: This affiliation will be effective when all parties have signed this agreement. It will be effective for a three year period ending August 31, 2007; provided however, this agreement may be terminated art any time for cause and/or violations of the terms herein. Furthermore, either party may terminate this agreement upon ninety(90) days written notice to the other party. P, new agreement may be initiated if major changes or revisions in policies are considered necessary by college or the affiliate. Responded to clientele contacts regarding agriculture,horticulture, natural resource, and 4-H related questions. Responses were conveyed through individual contact methods such as office visits, phone calls, email, fax, etc. Completed grant application for Calhoun County Community Garden Association concerning fence repair of damage from Hurricane Claudette in July 2003. Application was delivered to the Dow plant near Long Motto. 5 County Holiday, Extension Office Closed. Departed for National Swine Registry`Summer Spectacular'to be held in Louisville, KY. 6-11 Summer Spectacular, Kentucky Fair & Exposition Center, Louisville, KY — 6 attended, 3 exhibitors, 3 projects exhibited, 3 volunteers. Three (3) purebred gilts (Yorkshire, Hampshire, & Duroc) were exhibited at the NJSR national show. Exhibitors also competed in additional competitions such as showmanship, skillathon, and live animal evaluation. Furthermore, the exhibitors and volunteers viewed new boars that were on exhibition with plans to purchase semen to improve their breeding program. 7 Turn Row Meeting, FM 35 Soybean Field — 14 attended. Meeting was organized on short notice due to bacteriological damage developing in soybean fields across the county. Meeting notices were distributed to all agricultural related businesses. Dr. Steve Livingston and Dr. Tom Isekeit presented at the meeting. 12 Make-up Holiday for July 5. 13 Farm Visits (2), SH316 & FM1679 — :I attended. Setup Mexican Rice Borer (MRB) pheromone traps at Hubbard Faun near organic rice fields. Calhoun County is again participating in a trapping study coordinated through the Beaumont Experiment Station. Met with soybean producer at his farm to assist with chemical applications over soybeans. IPM Agent attended both farm visits. Home Visit (1), Six -Mile Area — 2 attended. Assisted homeowner with identifying insect and fungal damage in home landscape and offered advise on treatment options. 14 Office Management. Responded to clientele contacts regarding agriculture,horticulture, natural resource,and4-H related questions. Responses were conveyed through individual contact methods such asoffice visits, phonecalls, email, fax, etc.' CCF Steer & Heifer Progress Show, Show Barn— 6 exhibitors, 24 volunteers, 7 steers; 2 heifers. Calhoun youth who entered steers and heifers for the October Fair participated in a mandatory progress show where they were instructed on showmanship. 15 Budget Hearing, Calhoun County Courthouse — 8 attended. Extension Agents presented budget request for FY2004 to the Calhoun County Commissioners Court. Project Visit (1), Six -Mile Area— Checked swine projects for proper nutrition and care. * Junior Livestock Committee, Agriculture Building — 8 attended. Committee discussed the upcoming show and volunteer superintendents gave reports on progress concerning their divisions. 16 Office Management. Project Visit (1), Six -Mile Area — Checked market swine projects for proper nutrition and housing.* Farm Visit (1), Foester Road — 6 attended. Harvested grain sorghum variety trial that was established in March. Garst Seed Company provided assistance with weighing the harvest and calculation of moisture. 20 Office Management. Responded to clientele contacts regarding agriculture,horticulture, natural resource, and4-H related questions. Responses were conveyed through individual contact methods such as office visits, phone calls, email, fax, etc. Project Visit (2), Seadrift — 3attended. Assisted market steer exhibitor with showmanship and • nutrition. A market Iamb project was also viewed and feeding was discussed with the parent. 21 Home Visits (2), Port Lavaca-2 attended. Offered advise on propagation of turf around a local business building. The second home visit concerned barklice in a home landscape. Traveled to Victoria with FCS Agent to discuss administrative planning conference with Youth/4-H Coordinator. The use of P/I worksheets as well as accurate repotting was discussed. Due to the addition of P/I worksheets to position, monthly reporting using the monarch system will no longer be a responsibility of this position. Administrative Planning Conference, Youth/4-H Coordinators Office-3 attended. Discussed effective plannnn andthe use ofP/I worksheets to achieve effective planning and repotting of youth educational activities. In return for completing these worksheets, reporting using the monarch system will no longer be a responsibility of that position. Professionalism was also discussed in addition to inter -office communication. * Ranch Visit (1), FM 2541 — 2 attended. Met with local hay producer to establish Fall Armyworm (FAW) pheromone traps. These traps will be used to monitor atmyworm mating flights in hopes to better predict worm infestations. 22 Farm & Ranch Visits (3), Six -Mile Area, Sanders Road, & FM 2235 — 2 attended. Made Farm and Ranch visits to check test plots (cultivation trial and brush control strip test). Visited home of cooperator to discuss cultivation test which was not implemented due to excessive rainfall that caused too long ofa delay. IPM agent attended these visits. 23 Office Management.
