2006-10-30 Special October Meeting Held October 30, 2006 THE STATE OF TEXAS !i !i COUNTY OF CALHOUN. !i BE IT REMEMBERED, that on this 30th day of September A.D., 2006 there was begun and holden in the Commissioners' Courtroom in the Memorial Medical Plaza in the City of Port Lavaca, said County and State, at 10:00 A.M., a Special Term of the Commissioners' Court within said County and State, and there were present on this date the following members of the Court, to- wit: Michael J. Pfeifer Roger C. Galvan Michael J. Balajka Neil E. Fritsch . Shelby Sebastian County Judge Commissioner, Precinct #1 Commissioner, Precinct 2 Commissioner, Precinct 3 Deputy County Clerk Absent from the meeting was Commissioner Kenneth W. Finster. Thereupon the following proceedings were had: COUNTYCHOICE SILVER PLAN OFFERED BY THE TEXAS ASSOCIATION OF COUNTIES FOR CALHOUN COUNTY EMPLOYEES: A Motion was made by Judge Pfeifer and seconded by Commissioner Balajka to accept the CountyChoice Silver Plan offered by the Texas Association of Counties for Calhoun County Employees. No other Medicare supplement or Medicare Advantage program will be offered to these retirees. · Agree that this coverage will not be offered to any ACTIVE employee, regardless of age. Box 2131 Austin, TX 78768 Billing Option Descriptions DIRECT BILLED: LIST BILLED: SPLIT BILLED: Retiree is billed monthly. Employer agrees to pay monthly billing. Employer must indicate the contribution levels for Employer and for Retirees. Bills will be created and sent to the Employer for the Employer portion and to the Retiree for any remaining balance. To ensure that each eligible retiree has the opportunity to enroll, the Employer must provide a list, in Excel format if possible, of all current retirees. The list should include- name, mailing address, and date of birth. If the Employer does not maintain this list, a copy may be requested from TCDRS. The Employer must also provide T AC regular updates of new retirees so that we may provide the enrollment ForTAC use only: CountyChoice Silver Entity code: 2 Interlocal rec'd: 1 D/2006 materials to those individuals as welL TAC and Aetna will work together to mail Enrollment Kits to each retiree at the addresses provided. CountyChoice Sliver 3 1012006 ACCOUNTS ALLOWED - HOSPITAL: Claims totaling 2,354,461.06,were presented by the County Treasurer and after reading and verifying same, a Motion was made by Commissioner Balajka and seconded by Commissioner Fritsch that said claims be approved for payment. Commissioners Galvan, Balajka, Fritsch and Judge pfeifer all voted in favor. ACCOUNTS ALLOWED - INDIGENT HEALTHCARE: Claims totaling $112,170.31 were presented by the County Treasurer and after reading and verifying same, a Motion was made by Commissioner Balajka and seconded by Commissioner Fritsch that said claims be approved for payment. Commissioners Galvan, Balajka, Fritsch and Judge Pfeifer all voted in favor. PETITION TO VACATE LOTS 1-9 AND PART OF LOTS 10-12. BLOCK 13. LOTS 1. 2. 11. 12 AND PARTS OF LOTS 3 &. 10. BLOCK 12 AND PART OF THE ALLEY IN BLOCK 12 &. 13. TILKE AND CROCKER FIRST ADDITION TO ALAMO BEACH AND APPROVE FINAL PLAT OF BAYSIDE ESTATES. A 3.18 ACRE TRACT: A Motion was made by Commissioner Galvan and seconded by Commissioner Balajka to approve the Petition to Vacate Lots 1-9 and part of Lots 10-12, Block 13, Lots 1, 2, 11, 12 and Parts of Lots 3 & la, Block 12 and Part of the Alley in Block 12 & 13, Tilke and Crocker First Addition to Alamo Beach and Approve Final Plat of Bayside Estates, a 3.18 Acre Tract.
